Frequently asked questions

Does this appoint a healthcare agent?

No. This product records treatment wishes. Use the separate Advance Healthcare Directive and Medical Power of Attorney to appoint a decision-maker.

Is this a DNR or POLST?

No. Clinician and EMS medical orders are separate state-specific instruments.

Why do some states ask different questions?

Some states have a pinned official or statutory living-will body, so Locke asks that form's elections instead of translating a national checklist. Other states, including New Jersey, stay on the base treatment instrument plus that state's execution, pregnancy, and medical-order rules because the statute does not require one official form.

What if my free text conflicts with a treatment choice?

Final generation is blocked until the conflict is corrected. Locke checks treatment-directed free text and out-of-scope attempts to appoint an agent.

Does the price appear on the signed document?

No. Catalog price lives in the product tier table. The living will itself does not print a dollar amount, so a later price change does not require rewriting the instrument.
